register_now_blue_signWe welcome all patients new to the area who wish to register with this practice. We have a practice boundary which you can ask receptionist for and we can only register patients living within this boundary.

If you decide to register with the practice we ask that you complete a registration form and questionnaire and bring 2 forms of ID with you (one being a photo ID). You then must make an appointment to register with our healthcare assistant - Please note that until you have completed your registration form, have the correct form of ID and come in to see our healthcare assistant you will not be registered.

If we cannot accept you onto the list you will be given a full written explanation outlining the reason/s for this. New patients are registered with the practice and not a specific doctor. However, at the time of registration you have the right to ask to be registered with a specific doctor and we will make every effort to see that you see this doctor whenever possible. Due to holidays or sickness this may not always be possible but we will always offer an alternative if you wish to be seen urgently.

Alternatively, you can wait until an appointment is available for the doctor or your choice. All newly registered patients over the age of five years will be offered an appointment for a registration medical with our healthcare assistant.




When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us.
